INSURANCE & PAYMENT INFORMATION
We accept most major insurance providers, and we will do our best to verify your coverage during the registration process. However, please note that detailed information about your benefits, including co-pays, deductibles, and out-of-pocket costs, may not be fully confirmed until after your insurance is processed. If you have specific questions about your coverage or whether psychological evaluations are included, we recommend contacting your insurance provider directly for the most accurate and detailed information. If you do not have insurance or prefer to pay out-of-pocket, we provide payment information including self-pay rates below. If your insurance provider is not listed below, we may still be able to verify your coverage—please contact us to check whether we accept your plan.

PAYMENT INFORMATION
To complete your payment, please visit the Billing tab in your Patient Portal at https://www.therapyportal.com/p/apexpsych/. Alternatively, you can make a payment over the phone or by check, made payable to Apex Psychological Services.
The out-of-pocket cost for the full evaluation starts at $1,500, with the final amount depending on the specific tests administered during your evaluation. Please note: Insurance claims may take some time to process, and this could delay the final determination of your total cost.
Self-Pay: If you do not have insurance or prefer to pay out-of-pocket, we require a payment of at least 50% of the total evaluation cost at the time of the testing appointment. The remaining balance will be due upon receipt of the report.
Payment Plans: We accept partial payments at any time throughout the process to make it more manageable for you. All payments should be completed prior to or at your final session. However, we understand that insurance claims can take time to process, and any remaining balance due after insurance has been applied will be your responsibility.

Insurance Billing: If you have insurance and prefer to use your benefits, we will submit the necessary paperwork to your insurance provider on your behalf. Please be aware that coverage for psychological evaluations may vary, and you may be responsible for co-pays, deductibles, or any uncovered services. Due to the time required for insurance claims to be processed, there may be a delay in determining your final cost.

Cancellation Policy: If you need to cancel or reschedule your appointment, please notify us at least 24 hours in advance to avoid any cancellation fees. All clients will be asked to put a card on file at the time of registration.
